Forms & Docs

Completing your paperwork before you arrive keeps check-in brief and gives our team time to review your history ahead of your consultation. New patients should complete the New Patient Intake and Medical History Form; both help us understand your symptoms and any prior treatment before your visit. The Privacy Policy Notice explains how your medical information is handled. If you have insurance, bring your card and a photo ID to your first visit.

Appointments & Insurance

Do you accept my insurance?

We are in-network with most major PPO insurance providers, including Blue Cross, Blue Shield, Aetna, and Cigna. Our billing department will verify your specific benefits prior to your consultation.

How long does an initial consultation take?

Please allocate approximately 60 to 90 minutes for your first visit. This includes a detailed medical history review, diagnostic imaging (if necessary), and a thorough discussion of personalized treatment pathways.

Treatments & Procedures

Is Balloon Sinuplasty painful?

Balloon Sinuplasty is minimally invasive and performed in-office under local anesthesia. Most patients report feeling pressure rather than pain, and experience a rapid return to normal activities within 24 to 48 hours.

Snoring & Sleep Apnea

Do you treat snoring and sleep apnea?

Yes. We offer minimally invasive treatments for snoring and sleep apnea conditions. The right approach depends on your symptoms and evaluation, which we review together at your consultation. Read more about our snoring treatments and sleep apnea options.

Do you offer alternatives to CPAP?

For appropriate patients, we discuss minimally invasive alternatives to CPAP as part of a sleep apnea evaluation. Which options are suitable depends on your diagnosis and anatomy. See our CPAP alternatives page for more detail.
